The Archive offers an assisted search service, available to institutions and private individuals. The service is provided free of charge in, for example, social cases, or cases of claims for damages; otherwise a fee is charged, as specified in the price list.
We carry out assisted searches on the basis of written requests, submitted in person, or by post, fax, or e-mail, or oral requests, duly recorded, as stipulated in the Code of Administrative Procedure. This record should be signed by both the person submitting the request and the member of the Archive's staff by whom it is taken. Requests submitted in electronic form should bear a secure electronic signature, authenticated by a valid qualified certificate, observing the rules set out in the legislation on electronic signatures, and should also contain data complying with the established format, as contained within the specimen request provided in separate legislation, should the legislation in question require requests to be submitted in accordance with a stipulated form.
The Code of Administrative Procedure makes no provision for the submission of requests by telephone.
As a public administration body, the Archive is required to acknowledge receipt of a request if the person submitting it so requires.
In line with the Code of Administrative procedure, a request should contain, at the very least:
- the forename and surname of the person submitting the request;
- the address of the person submitting the request;
- the substance of the request;
- the signature of the person submitting the request;
- any other data required by other relevant legislation.
The fee due for each hour of work begun on a search, together with that for making copies, duplicates and extracts, equivalent, for the purposes of carrying out legal transactions, to certificates, is collected in line with the price lis t of services provided by the State Archive in Kalisz.
In line with the Code of Administrative Procedure, authenticated copies, duplicates and extracts from the records are issued on request, if:
- the law requires official confirmation of established facts or legal status;
- the person is requesting the authentication as a result of a legal interest in having an official confirmation of established facts or legal status.
The appropriate stamp duty is also payable for making authenticated copies, duplicates and extracts, in accordance with the Act of 16th November 2006 (Dz. U. [Journal of Laws] 225, No , item 1635) and the Minister of Finance's Directive of 21st December 2006 (Dz. U. 2006, No 246, item 1804).
